The main symptom of dysmenorrhea is pain. It occurs in your lower abdomen during menstruation and may also be felt in your hips, lower back, or thighs. Other symptoms may include nausea, vomiting, diarrhea, lightheadedness, or general aching. For most women, the pain usually starts shortly before or during their menstrual period, peaks after 24 hours, and subsides after 2 days. Sometimes clots or pieces of bloody tissue from the lining of the uterus are expelled from the uterus, causing pain.

Dysmenorrhea pain may be spasmodic (sharp pelvic cramps at the start of menstrual flow) or congestive (deep, dull ache). The symptoms of secondary dysmenorrhea often start sooner in the menstrual cycle than those of primary dysmenorrhea, and usually last longer.

Primary dysmenorrhea is thought to be caused by excessive levels of prostaglandins, hormones that make your uterus contract during menstruation and childbirth. Its pain probably results from contractions of your uterus that occur when the blood supply to its lining (endometrium) is reduced. Usually, the pain will happen only during menstrual cycles in which an egg is released. The pain may progressively become worse as endometrial tissue shed during a menstrual period passes through the cervix, particularly when the cervical canal is narrow.

Other factors that may make the pain of primary dysmenorrhea even worse include a uterus that tilts backward (retroverted uterus) instead of forward, lack of exercise, and psychological or social stress.

Secondary dysmenorrhea may be caused by a number of conditions, including:

  • fibroids - benign tumours that develop within the uterine wall or are attached to it
  • adenomyosis - the tissue that lines the uterus (called the endometrium) begins to grow within its muscular walls
  • a sexually transmitted disease (STD)
  • endometriosis - fragments of the endometrial lining escape the uterus and become implanted on other pelvic organs
  • pelvic inflammatory disease (PID), which is primarily an infection of the fallopian tubes, but can also affect the ovaries, uterus, and cervix
  • an ovarian cyst or tumour

Home Remedies

Warm a small amount of olive oil, cool and rub on stomach. Lie down for 15-20 minutes.

Boil some brown sugar and ginger in water with low heat, cool and drink.

Red raspberry is known to strengthen the uterus, helps decrease heavy menstrual flow and relaxes muscles. Take as a tea, and drink one or two cups per day.

Herbal Remedies

Licorice Root - Used in ancient Chinese medications for female reproductive problems, this herb helps control water retention and breast tenderness. It also decreases symptoms associated with the fluctuation of hormones such as what occurs in menopause.

Black Cohosh - Also used to help the symptoms associated with menopause, this powerful herb helps relieve cramps, and helps with pain as well. You must take this herb for a few weeks for it to be effective.

Kava-Kava - A calming herb, kava-kava helps ease anxiety. It also has pain-relieving effects similar to aspirin. It is recommended to start taking this several days before your period and through your period.

Put some oregano leaves in boiled water and leave for 5 minutes, then drink. Lie down and rest for a few minutes.

Wellness Program

Cut out caffeine. The caffeine in coffee, tea, cola, and chocolate can contribute to menstrual discomfort by making you nervous, says Dr. Budoff. Go caffeine-free. The oils in coffee also may irritate your intestines.

Abstain from alcohol. If you tend to retain water during your period, alcohol will only add to your problems, says Dr. Lark. Don't drink, she advises. Or if you must, limit yourself to a glass or two of light wine.

Don't do diuretics. Many women think diuretics are great for reducing menstrual bloating, but Dr. Lark advises against them. Diuretics have the ability to take important minerals from the body along with the water. Instead, she advises, reduce your intake of water-retentive substances like salt and alcohol.

Warm up. Warmth will increase your blood flow and relax your muscles, especially important in your cramped and congested pelvic area, says Dr. Budoff. Drink lots of hot herbal tea or hot lemonade. Also, put a heating pad ofr hot-water bottle on your abdomen for a few minutes at a time.

Take a brisk walk. Walk or engage in some form of moderate exercise at all times, but especially before your period. You'll feel better when it arrives

Sex. Making love with orgasm is great for relieving cramps. The vigorous muscle action moves blood and other fluids away from congested organs, relieving pain.
